  Periodicals Borrowing for Faculty

Procedure:

a. The faculty brings the journal(s) to the staff at the Information Desk.
b. The library staff at the Information Desk must complete a Faculty Periodical. Borrowing Form for each of the issue borrowed.
c. File the white copy of the Faculty Periodicals Borrowing Form at the Information Desk, and a duplicate yellow copy is inserted in the periodicals and handed to the faculty. Instruct the faculty to stop at the Circulation Desk, show the completed Faculty Periodicals Borrowing Form, and ask to de-sensitize the journals before leaving the Library.
d. Remind the faculty that the loan period is for 24 hours, and when returning the journals to stop at the Circulation Desk to re-sensitize the journals before returning them to the Information Desk.
e. The faculty stops by the Circulation Desk to re-sensitize the journal(s) before returning them to the staff at the Information Desk.
f. The staff at the Information Desk pulls and removes the white copy of the Faculty Periodicals Borrowing Form on file at the Information Desk, and then returns the journal(s) to the Periodicals Room.
g. Make sure usage statistic is recorded for the journal(s).
h. Once a month, the staff at the Information Desk is to check the Faculty Periodicals Borrowing Forms on file at the Desk to make sure that the journals are returned.
i. Check the shelves to make sure issue(s) is/are still not accounted for.
j. Information Desk staff notifies the Periodicals Librarian for overdue journals.
k. The Periodicals Librarian sends out overdue notice(s) to the faculty requesting that the journals be returned immediately.

Returning

h. The faculty must stop by the Circulation Desk to re-sensitize the journal(s) before returning them to the staff at the Information Desk.
l. The staff at the Information Desk pulls and discards the white copy of the Faculty Periodicals Borrowing Form on file at the Information Desk.
m. Returns the journal(s) to the Periodicals Room.
n. Make sure usage statistic is recorded for the journal(s).
o. Once a month, the staff at the Information Desk is to check the Faculty Periodicals Borrowing Forms on file at the Desk to make sure that the journals are returned.
p. Check the shelves to make sure issue(s) is/are still not accounted for.
q. Information Desk staff notifies the Periodicals Librarian for overdue journals.
r. The Periodicals Librarian sends out overdue notice(s) to the faculty requesting that the journals be returned immediately.
s. If the overdue journals are not returned by the specified date, the Periodicals Librarian will bring the matter to the attention of the Library Director, who will then deal with the issue on the deans' level.
